Well, since you asked...



1. The original image.

2. In a seperate layer, expand the background to cover the stuff you don't want.

3. Put the original image in a layer on top of that one, and carefully mask out the parts that are replaced by the background (umbrella, purse, etc).

4. Get rid of the stuff you don't want that's on top of the woman (purse straps, umbrella handle).

5. Composite in the new stuff and mask edges.

6. Add the arms in on top. Mask edges. Add shading and blend everything together.

I did all this without any fancy plugins. Just a lot of layers and masking. That Extensis Mask Pro software looks pretty cool, though. I think I'll try the demo.
